#39: Delta Air Lines Flight 1141 (1988)
Taking off from Dallas–Fort Worth, the Boeing 727 failed to lift properly and crashed just beyond the runway, breaking apart and catching fire. Of the 108 people onboard, 14 were killed and many more seriously injured in the chaotic, fiery aftermath.

The investigation found that the crew failed to extend wing flaps and slats for takeoff, and cockpit recordings revealed unprofessional chatter before the accident. The crash led to tighter cockpit discipline rules and reinforced the importance of pre-flight checklists in preventing preventable disasters.
