#26: Spanair Flight 5022 (2008)
Attempting to take off from Madrid, an MD-82 failed to climb and veered off the runway before bursting into flames. Of the 172 people onboard, 154 were killed in the fiery crash, one of Spain’s worst aviation disasters.

The investigation revealed that the plane took off without proper wing flaps extended, and a safety warning system failed to alert the crew. The accident raised questions about airline oversight and safety checks in Europe. For Spain, Flight 5022 remains one of the most painful and controversial crashes in modern memory.
